FEES
Realize Behavioral Health provides addiction counseling and support for individuals, and families & loved ones. We can also conduct third-party assessments & evaluations for court, employment, or licensing boards.
If you have a PPO insurance plan with out-of-network benefits you may be eligible for full or partial reimbursement for some of our services. Please fill out the insurance verification form and we can verify your insurance benefits in advance and let you know the expected coverage amounts and potential deductibles.
All service fees are collected on the day services are provided. We cannot guarantee if any insurance company will provide reimbursement for services, even if our services are covered by your policy.
Sliding Scale option available in a limited capacity for counseling and therapy, based on annual household income and the clinicians availability for reduced rate clients. Call our office today if you would like to discuss our sliding scale accessibility.

INSURANCE
Regarding insurance, please know that we are an out-of-network provider and collect payment at the time of service. We will provide you with all the necessary paperwork to submit to your insurance company to request reimbursement.
